Abstract

AIMS:

Currently, there is little information about the need for peri-operative blood transfusion in patients undergoing shoulder arthroplasty. The purpose of this study was to identify the rate of transfusion and its predisposing factors, and to establish a blood conservation strategy.

DISCUSSION:

We found that transfusion was needed less frequently than previously described for shoulder arthroplasty. Patients with a pre-operative haematocrit < 39.6% should be advised that there is an increased risk for blood transfusion, while those with a haematocrit above this level are unlikely to require transfusion.

TAKE HOME MESSAGE:

The rate of transfusion after shoulder arthroplasty is under 5%, and those with a pre-operative haematocrit greater than or equal to 39.6% have a very low likelihood (< 1%) of requiring a transfusion. Cite this article: Bone Joint J 2016;98-B:224-8.

 

Resumen

OBJETIVOS:

Actualmente, hay poca información acerca de la necesidad de transfusión perioperatoria de sangre en pacientes sometidos a artroplastia de hombro. El propósito de este estudio fue identificar la tasa de transfusión y sus factores predisponentes, y establecer una estrategia de conservación de la sangre.

DISCUSIÓN:

Encontramos que la transfusión se necesita con menos frecuencia de la que se ha descrito anteriormente para la artroplastia de hombro. Los pacientes con un hematocrito preoperatorio <39.6% deben ser advertidos de que existe un mayor riesgo de transfusión de sangre, mientras que aquellos con un hematocrito por encima de este nivel es improbable la necesidad de transfusión.

LLEVAR A CASA MENSAJE:

La tasa de transfusión después de la artroplastia de hombro está abajo del 5%, y aquellos con un hematocrito preoperatorio mayor que o igual a 39,6% tiene una probabilidad muy baja (<1%) de requerir una transfusión.
