Componentes glenoideos sin cemento versus componentes cementados en la artroplastia total de hombro convencional: análisis del Registro Nacional de Reemplazo de Articulaciones de la Asociación Australiana de Ortopedia.

El aflojamiento glenoideo es un modo común de falla después de la artroplastia total de hombro (TSA). Se han introducido nuevos componentes glenoideos sin cemento para promover la fijación biológica con el objetivo de disminuir el aflojamiento glenoideo. Se dispone de datos limitados que comparan las tasas de revisión entre la fijación glenoidea cementada y no cementada en TSA. El objetivo del estudio fue comparar las tasas de revisión de los componentes glenoideos de diseño cementado y sin cemento utilizados en la TSA convencional realizada para el diagnóstico de osteoartritis. El objetivo secundario fue comparar varias subclases de componentes glenoideos.

Los componentes glenoideos sin cemento en la TSA convencional tuvieron una tasa de revisión significativamente más alta que los componentes glenoideos cementados. Las tasas de aflojamiento entre los componentes glenoideos cementados y no cementados fueron similares. El diseño y la fijación de los glenoides son consideraciones importantes al seleccionar una prótesis para TSA.

Abstract

BACKGROUND:

With an increasingly large number of patients undergoing total shoulder arthroplasty (TSA) combined with increased requirements for public reporting of patient outcomes, there is a greater need to better understand the underlying factors related to patient satisfaction. The purpose of this study was to compare patient demographics, nonorthopedic comorbidities, patient-reported outcome scores, and range of motion of patients who reported being either satisfied or dissatisfied with their procedure at midterm follow-up.

CONCLUSION:

Dissatisfied patients had significantly lower improvements in pain, function, and range of motion. Furthermore, a change in ASES score <12 points was associated with a 19-fold increase in the risk of being dissatisfied after TSA.

Resumen
ANTECEDENTES:

Con un número cada vez mayor de pacientes sometidos a artroplastia total de hombro (TSA) combinado con mayores necesidades de información pública de los resultados de los pacientes, hay una mayor necesidad de comprender mejor los factores subyacentes relacionados con la satisfacción del paciente. El propósito de este estudio fue comparar los datos demográficos del paciente, comorbilidades no ortopédicas, puntuaciones de resultados informados por los pacientes, y el rango de movimiento de pacientes que informaron estar satisfechos o insatisfechos con su procedimiento en el seguimiento a mediano plazo.

CONCLUSIÓN:

pacientes insatisfechos tuvieron mejoras significativamente más bajos en el dolor, la función y el rango de movimiento. Además, un cambio en la puntuación ASES <12 puntos se asoció con un aumento de 19 veces  el riesgo de ser no estar satisfecho después de una TSA.

Abstract

BACKGROUND:

Primary glenohumeral osteoarthritis is a common indication for shoulder arthroplasty. Historically, both total shoulder arthroplasty (TSA) and hemi-shoulder arthroplasty (HSA) have been used to treat primary glenohumeral osteoarthritis. The choice between procedures is a topic of debate, with HSA proponents arguing that it is less invasive, faster, less expensive, and technically less demanding, with quality of life outcomes equivalent to those of TSA. More recent evidence suggests TSA is superior in terms of pain relief, function, ROM, strength, and patient satisfaction. We therefore investigated the practice of recently graduated orthopaedic surgeons pertaining to the surgical treatment of this disease.

CONCLUSIONS:

The findings of this study are at odds with the recommendations in the current clinical practice guidelines for the treatment of glenohumeral osteoarthritis published by the American Academy of Orthopaedic Surgeons. These guidelines favor using TSA over HSA in the treatment of shoulder arthritis. Further investigation is needed to clarify if these practice patterns are isolated to recently graduated board eligible orthopaedic surgeons or if the use of HSA continues with orthopaedic surgeons applying for recertification.
